You can now download all of the necessary forms in one file. Fill out the Patient Information Sheet, Consent to Treatment, and Electronic Payment Authorization (if you plan to pay using a credit card) portions of the PDF. The HIPPAA form is simply for your information and requires no action on your part. Once completed, print and sign the Consent to Treatment and Electronic Payment Authorization Form. Please e-mail them back to us as an attachment.
